Berwyn Township was established by the Illinois Legislature in 1922 as one of the few coterminous townships in the state. The Berwyn Public Health District was established at the same time. The Township and Public Health District moved to their present headquarters at 6600 W. 26th Street in 1939. Built for $50,000, construction of the building was made possible by federal grants under the Works Progress Administration (WPA).

The Berwyn Public Health District provides immunizations free of charge to children that are uninsured or enrolled in Medicaid through the Vaccines For Children Program. Call for additional information and appointments for immunizations.

HEALTH DISTRICT SERVICES INCLUDE:

• Sanitation Department: restaurant inspections, new business inspections, complaint monitoring
   and  follow-up.

• KidCare Assistance: assist applicants in filling out and filing forms for KidCare, an Illinois sponsored 
   free or low cost medical insurance for kids and pregnant women. Our office is also an Access To Care
   registration site, we can assist our residents in completing applications for this program.

• Vital Records Office: recording of Berwyn births and deaths, maintenance of records back to the early
   1900's.

• Nursing Department: low cost school physicals, immunization clinic, infant, toddler, school age, and
   adults, by appointment for Berwyn residents, blood pressure screening, tetanus and flu inoculations 
   (adult & children), hearing screenings, vision screenings, diabetes screenings, special cholesterol
   screenings, agency referral service, lead screening referrals and follow-up.

• Dental: school fluoride rinse program; dental education and nutrition program in Berwyn schools; in
   school dental exams provided through partnership with the Children’s Clinic of Oak Park.

• Special Services: medical waste disposal - special sharps program for residents, RX Take back
   Program allows for collection of non-narcotic unused or expired prescription drugs. cooperation with
   other agencies.

TOWNSHIP SERVICES INCLUDE:

• Issuance of Temporary Handicap Parking Placards

TOWNSHIP ASSESSOR'S OFFICE

Provides assistance to:

• Homeowners in completing applications and answering questions regarding homeowner's
  exemption, senior exemption, senior freeze and disability exemption.

• Property owners in obtaining duplicate tax bills.

• Residents in submitting name or address changes to Cook County Treasurer's Office.

• Homeowners in submitting appeals to Cook County Assessor's Office / Cook County Board
  of Review to request a reduction of assessed property valuation.

GENERAL ASSISTANCE OFFICE

• General Assistance is provided to eligible unemployed adults that meet the program requirements and
   are not eligible for other government sponsored assistance. While receiving General Assistance,
   individuals must adhere to the program guidelines. Able bodied individuals must be actively seeking  
   work.
